We have changed our enrollment procedures. We'd like to make things easier for our caregivers.

MEDICAL -Our nurse will  contact your physician.   The physician can then forward any pertinent  medical information. TB testing will not be required, only if it is recommended by your physician.

DOCUMENTATION NEEDED for attendance

• Durable Medical Power of Attorney
• Advanced Medical Directives
• List of all Medications

Our enrollment questionaires can be picked up at our reception desk or mailed to you.
